Organization Camp Day 3: Evaluation Folder Form

Happy Friday! Today I’m sharing with you something I have never lived without as a School Psych. I have been using this form in some capacity for 7 years now. I love it. It helps me see at a glance what I need to complete for an evaluation, what I’ve collected/completed, and what I still need to do.

What is it?  A cover sheet that you can staple to the front cover of the file folder you keep your evaluation in with all your need to know info “at a glance”.

How do I use it?  The first thing I do after I get a new eval or start a triannual is grab a brightly colored folder and staple this to the front of it. I write out what my plan for testing is, then gather everything and send it. I also try to make a copy of the actual signed testing form and staple that to the inside cover, in case I forget anything.

Create it: Make a list of the things you gather for evaluations, and create a general list that you can fill in.

Or Use Mine: You can the version I have been using for the last several years, and an alternate for those who did academic testing, in my keep calm freebies. or an updated version in my TPT store.

So, in closing, your mission is to create a form that can help you easily organize what testing you are doing, and check off when you complete it. Keep a bunch in a folder with your protocols for easy access!
